The procedure described by CPT® Code 57155 involves the insertion of uterine tandem and/or vaginal ovoids specifically for the purpose of delivering clinical brachytherapy. In this context, a uterine tandem refers to a long, thin applicator that is inserted into the uterus, while vaginal ovoids are rounded applicators that are placed in the vagina. Both of these devices are initially empty and are designed to be loaded with a radiation source after their placement by a qualified surgeon. The primary goal of this procedure is to administer high-dose radiation directly to malignant neoplasms located in the uterus or vagina, thereby maximizing the therapeutic effect on the cancerous tissue while minimizing exposure to surrounding healthy tissues. The use of a uterine tandem is particularly indicated for treating neoplasms situated in the lower uterus and the uterine cervix, whereas vaginal ovoids are primarily utilized for addressing neoplasms found in the vaginal cuff and the upper third of the vagina. It is common practice for both the uterine tandem and vaginal ovoids to be inserted during the same procedure, allowing for a comprehensive approach to brachytherapy. The physician is responsible for determining the precise number and placement of the tandem and/or ovoids based on the specific characteristics of the tumor and its location. The tandem is typically inserted directly at the tumor site in the lower uterus or cervix, while the vaginal ovoids are positioned laterally on either side of the vaginal fornix. Following the placement of these applicators, a separate procedure known as afterloading is performed, where the radiation source is introduced into the applicators to deliver the intended therapeutic radiation dose.
