Common Language Description

The procedure described by CPT® Code 57426 involves the revision and potential removal of a prosthetic vaginal graft using a laparoscopic approach. A prosthetic vaginal graft is a surgical implant used to support the vaginal walls, which may require revision due to complications such as infection, stricture, or erosion of the graft material through the surrounding tissues. The laparoscopic technique is minimally invasive, allowing for smaller incisions and reduced recovery time compared to traditional open surgery. During the procedure, a small incision is made below the umbilicus to insert a laparoscope, which is a specialized instrument equipped with a camera to provide visualization of the surgical area. Additional small incisions are created for the insertion of trocars, which facilitate the introduction of surgical instruments into the abdominal cavity. The surgeon elevates the vagina using an instrument placed within the vaginal cavity, allowing for better access to the graft. The peritoneum, which is the lining of the abdominal cavity, is then opened to inspect the graft. The surrounding tissue is carefully dissected to assess the condition of the graft and to determine whether all or part of it needs to be removed. If any graft material is left, it is rearranged along with the endopelvic fascia and vaginal tissue to ensure proper coverage of the vaginal apex. The procedure concludes with the reapproximation of the endopelvic fascia using sutures, followed by a layered closure of the vaginal epithelium and mucosa. Finally, the peritoneum is closed laparoscopically, and the surgical instruments are removed, with the portal incisions being closed to complete the operation.
