The procedure described by CPT® Code 62273 involves the injection of a blood or clot patch into the epidural space. This intervention is specifically utilized to address severe headaches that arise as a complication from a leak of spinal fluid into the epidural space. Such leaks can occur following procedures like epidural anesthesia or diagnostic and therapeutic spinal punctures. The process begins with the physician disinfecting the skin over the lower back and administering a local anesthetic to minimize discomfort. Following this, a separate site is prepared for venipuncture, where an intravenous catheter is inserted into a vein to facilitate the withdrawal of blood. The physician then carefully places an epidural needle into the epidural space, ideally near the location of the previous puncture. Blood is drawn from the venous catheter and subsequently injected into the epidural space. This blood injection serves to create a patch that effectively seals the cerebrospinal fluid leak, alleviating the associated headache. After the injection, the patient is typically advised to rest for approximately 30 minutes, allowing the blood to form a clot and establish a barrier over the leak, thereby promoting recovery and symptom relief.
