Extracapsular cataract removal is a surgical procedure aimed at treating cataracts, which are characterized by the clouding of the lens of the eye. This procedure involves the removal of the cloudy lens and the insertion of an intraocular lens (IOL) prosthesis to restore clear vision. The surgery is typically performed through an incision made at the corneoscleral junction, also known as the fornix, which allows access to the eye. During the procedure, a side port incision is created, and the pupil is dilated to facilitate the removal of the cataract. A circular tear is made in the anterior lens capsule, which is then opened to allow the insertion of a phacoemulsification probe. This device utilizes ultrasound technology to break up the cataract into smaller pieces, which are subsequently aspirated out of the eye. In some cases, manual techniques may be employed for cutting and suctioning the cataract. To protect the cornea during the procedure, a bubble of air is injected into the anterior chamber. In addition to the cataract removal and IOL insertion, this procedure also includes the placement of an anterior segment aqueous drainage device. This device is specifically designed to manage chronic or progressive open-angle glaucoma by facilitating the drainage of aqueous fluid from the eye, thereby reducing intraocular pressure. The drainage device is inserted without an extraocular reservoir and can be positioned in various locations, such as the trabecular meshwork, supraciliary space, or suprachoroidal space, depending on the specific needs of the patient. The use of a magnification lens, such as a gonioscope, aids in the precise placement of the drainage device at the angle of the anterior chamber. After the completion of the procedure, incisions may be closed with sutures, and a dressing is applied to the eye to promote healing.
