The CPT® Code 67028 refers to the procedure of intravitreal injection of a pharmacologic agent, which is classified as a separate procedure. This technique involves administering a medication directly into the vitreous cavity of the eye without the need for any surgical incision or removal of vitreous gel. The pharmacologic agents used in this procedure are typically medications that target specific ocular conditions, particularly those related to the retina. Commonly injected drugs include Lucentis (Ranibizumab), which is a monoclonal antibody fragment that inhibits vascular endothelial growth factor (VEGF) and is primarily utilized in the treatment of neovascular (wet) age-related macular degeneration (AMD) and macular edema resulting from retinal vein occlusion. Other medications such as Macugen (pegaptanib sodium) and Eylea (Aflibercept) are also VEGF antagonists approved for similar indications. Additionally, Avastin (Bevacizumab), although not specifically approved for this use, is sometimes administered off-label for the treatment of AMD via intravitreal injection. This procedure is typically performed in an outpatient setting, such as a physician's office, where a topical ophthalmic anesthetic is applied to minimize discomfort. The eye is then cleansed with an antiseptic solution, and an eyelid speculum is used to keep the eye open during the injection. The injection is performed using a fine 30-gauge needle, which is inserted into the mid-vitreous cavity to deliver the medication, after which the needle is withdrawn. Post-procedure care may include the application of antibiotic eye drops and, in some cases, patching the eye to protect it during the initial recovery phase.
