Official Description

Closure of the lacrimal punctum; by plug, each

Common Language Description

The procedure described by CPT® Code 68761 involves the closure of the lacrimal punctum through the insertion of a plug, which is a common treatment for dry eye syndrome. The lacrimal punctum is a small opening located at the inner corner of the eyelids that allows tears to drain from the eye into the nasal cavity. In cases where patients experience dry eye syndrome, this drainage can be excessive, leading to discomfort and irritation. To alleviate these symptoms, a plug is inserted into the punctum to block the drainage, thereby retaining moisture in the eye. Prior to the procedure, anesthetic eye drops or other forms of local anesthesia are applied to the conjunctiva, the membrane covering the eye, to minimize discomfort during the insertion of the plug. This procedure is distinct from other methods of punctal occlusion, such as thermocauterization, ligation, or laser surgery, which involve different techniques to achieve closure of the punctum. In this specific procedure, the punctum is carefully inspected and may be dilated if necessary to facilitate the insertion of the plug. The eyelid is gently pulled away from the eyeball to provide better access, and the plug is grasped with fine forceps and inserted into the punctum. The lid is then manipulated to ensure that the plug is positioned correctly, effectively closing the punctum and helping to manage the symptoms of dry eye syndrome.
