The CPT® Code 69145 refers to the excision of a soft tissue lesion located in the external auditory canal (EAC). This procedure is typically performed by a physician to address exostoses, which are bony growths that can develop in the EAC, leading to a gradual narrowing of the canal due to the deposition of bone on the canal wall. The process begins with an otoscopic examination, where the physician inspects the EAC to identify the presence of exostoses. Following this examination, an operating head is attached to the otoscope, allowing for the introduction of surgical instruments through the speculum. The area is then cleansed with a disinfectant to minimize the risk of infection. A medially based canal skin flap is created and reflected onto the tympanic membrane, providing access to the underlying structures. The exostoses are meticulously drilled away using specialized tools such as bone curettes and diamond burrs. After the excision, the surgical site is covered with the previously created skin flap and subsequently closed. In the context of CPT® Code 69145, the procedure specifically involves the removal of a soft tissue lesion from the EAC, along with a margin of healthy tissue to ensure complete excision. During this process, electrocautery may be employed to control any bleeding that occurs. The surgical wound resulting from the excision may either be closed or left open to allow for granulation, and the ear canal may be packed with sterile gauze to aid in the healing process.
