Common Language Description

Cholangiography and pancreatography are specialized imaging procedures utilized to visualize the bile ducts and pancreatic duct, respectively. These procedures are particularly important during biliary surgery, as they help identify issues such as stones, inflammation, or obstructions within the bile ducts and assess the pancreatic duct for any trauma. The process begins after the removal of the gallbladder, where a small catheter is inserted into the cystic duct. This duct is responsible for draining bile from the gallbladder into the common bile duct, which is a critical pathway for bile flow. The pancreatic duct, which also plays a vital role in digestion, joins the common bile duct just before the ampulla of Vater, a key anatomical landmark in the digestive system. To capture images of these structures, X-ray imaging is employed, utilizing indirect ionizing radiation. This method is effective because it can differentiate between various densities and compositions of human tissue, allowing some X-rays to be absorbed while others pass through. The result is a two-dimensional image that provides valuable insights into the anatomy and function of the bile and pancreatic ducts. To enhance the clarity and detail of these images, a contrast material is injected into the duct(s), which improves the visualization of the surrounding structures. The entire procedure is conducted under the direct supervision of a radiologist, who oversees the injection of the contrast material and interprets the findings, providing critical information to the surgeon. The CPT® Code 74300 is specifically designated for reporting the radiological supervision and interpretation of the initial cholangiography or pancreatography study performed intraoperatively, while additional image sets obtained during the same procedure are reported using CPT® Code 74301.
