The CPT® Code 78271 refers to a laboratory test that assesses the absorption of Vitamin B-12, also known as cobalamin, which is essential for several critical bodily functions. Vitamin B-12 plays a vital role in the production of red blood cells (RBCs), the replication of DNA and RNA necessary for cell development, and the proper functioning of the nervous system. This specific test is particularly useful in evaluating patients for pernicious anemia, a condition characterized by the inability to absorb Vitamin B-12 due to a lack of intrinsic factor, a protein produced in the stomach. The procedure involves administering an oral dose of Vitamin B-12 that is labeled with an isotope tracer, such as Cobalt 57 or 58, followed by a 24-hour urine collection to measure the amount of Vitamin B-12 absorbed by the gastrointestinal tract. If the results of the initial Stage I Schilling test (CPT® Code 78270) indicate abnormal absorption, the Stage II Schilling test (CPT® Code 78271) is performed.
