Common Language Description

Urobilinogen is a biochemical compound that results from the reduction of bilirubin, a substance produced during the breakdown of red blood cells. This reduction process primarily occurs in the intestine, where bilirubin is converted into urobilinogen. A small portion of urobilinogen is then reabsorbed from the intestine into the bloodstream and subsequently filtered by the kidneys, which is part of the enterohepatic urobilinogen cycle. The measurement of urobilinogen in urine can provide valuable insights into various health conditions. Elevated levels of urobilinogen in urine may indicate several medical issues, including biliary obstruction, hepatic infections, poisoning, or inflammation, as well as hemolytic anemia. Additionally, elevated levels can occur when the body is absorbing a large hematoma. Conversely, decreased levels of urobilinogen may be observed in cases of congenital enzymatic jaundice or when urine acidifying drugs are present. For different types of evaluations, specific CPT codes are utilized: CPT® Code 84578 is designated for qualitative evaluation, CPT® Code 84580 is used for quantitative, timed specimen evaluations, and CPT® Code 84583 is specifically for semiquantitative evaluations of urobilinogen in urine.

1. Indications

Urobilinogen testing is indicated for the evaluation of various medical conditions that may affect liver function or the breakdown of red blood cells. The following conditions may warrant the performance of this test:

  • Biliary Obstruction Elevated urobilinogen levels may indicate a blockage in the bile ducts, which can affect the normal flow of bile and lead to increased levels of urobilinogen in urine.
  • Hepatic Infection Infections affecting the liver can disrupt normal liver function and metabolism, resulting in altered urobilinogen levels.
  • Poisoning or Inflammation Certain toxins or inflammatory processes can impact liver function, leading to changes in urobilinogen production and excretion.
  • Hemolytic Anemia Conditions that cause increased destruction of red blood cells can lead to elevated levels of bilirubin and subsequently increased urobilinogen production.
  • Large Hematoma Absorption The presence of a significant hematoma that the body is in the process of absorbing may also result in elevated urobilinogen levels.
  • Congenital Enzymatic Jaundice Decreased levels of urobilinogen may be observed in patients with congenital enzymatic jaundice, indicating a metabolic issue.
  • Urine Acidifying Drugs The use of certain medications that acidify urine can lead to decreased urobilinogen levels.

2. Procedure

The semiquantitative evaluation of urobilinogen in urine involves a series of steps to ensure accurate measurement and analysis. The procedure typically begins with the collection of a urine sample, which should be done in a clean container to avoid contamination. Once the sample is collected, it is processed in a laboratory setting where it undergoes testing using specific reagents that react with urobilinogen present in the urine. The results are then interpreted based on the intensity of the color change observed, which correlates with the concentration of urobilinogen in the sample. This semiquantitative approach allows for the determination of whether urobilinogen levels are within normal ranges or if they are elevated or decreased, indicating potential underlying health issues.
