Common Language Description

An intermediate level of general ophthalmological services is provided to an established patient under CPT® Code 92012. This service is designed for patients who may present with a new condition or an existing condition that has become complicated by a new diagnosis or management issue. It is important to note that the new problem does not necessarily have to be related to the primary diagnosis. The evaluation process includes a comprehensive history or interval history, which allows the ophthalmologist to gather relevant information about the patient's current health status and any changes since the last visit. General medical observations are also made to assess the patient's overall health. The examination involves a thorough external ocular and adnexal examination, which assesses the structures surrounding the eye. The use of a slit lamp is integral to this process, as it provides a magnified view of the eye's anterior segment, allowing for detailed observation of the cornea, iris, and lens. Routine ophthalmoscopy is performed to examine the interior of the eye, including the retina and optic nerve, while biomicroscopy offers a more detailed view of the eye's structures. Tonometry is conducted to measure intraocular pressure, which is crucial for diagnosing conditions such as glaucoma. Additional techniques such as keratometry and retinoscopy may also be employed to evaluate the curvature of the cornea and refractive errors, respectively. To facilitate a thorough examination, mydriasis is induced, which involves dilating the pupils. This dilation enhances the physician's ability to visualize the ocular media and fundus during routine ophthalmoscopy. After completing the diagnostic evaluation, the ophthalmologist will either initiate a new diagnostic treatment program or continue with an established program. This may involve prescribing medications, arranging for additional special services, consultations, laboratory tests, or radiology services, ensuring that the patient receives comprehensive care tailored to their specific needs.
