Cardiovascular stress test using maximal or submaximal treadmill or bicycle exercise, continuous electrocardiographic monitoring, and/or pharmacological stress; supervision only, without interpretation and report

The CPT® Code 93016 refers to a cardiovascular stress test that utilizes either maximal or submaximal exercise on a treadmill or bicycle, along with continuous electrocardiographic (ECG) monitoring. This procedure may also involve pharmacological stress, which is a method used to simulate the effects of exercise on the heart when a patient is unable to perform physical activity. During the test, small plastic patches, known as electrodes, are affixed to specific locations on the patient's chest, abdomen, arms, and/or legs. These electrodes are connected to leads from the stress ECG device, which records the electrical activity of the heart throughout the procedure. Initially, a baseline ECG is obtained to establish the heart's normal rhythm and electrical activity. Following this, the exercise component of the test is initiated, where the patient's heart rate and blood pressure are continuously monitored. The test follows a staged stress protocol, progressively increasing the intensity of the exercise or pharmacological agent until the patient reaches their target heart rate or is unable to continue due to fatigue or other contraindications. It is important to note that CPT® Code 93016 is specifically designated for the supervision of the stress test without the inclusion of interpretation and reporting of the findings. For a complete procedure that includes interpretation and report, CPT® Code 93015 should be utilized. Additionally, CPT® Code 93017 is applicable for the tracing only, while CPT® Code 93018 is reserved for the interpretation and report only.
