Manual muscle testing is a clinical procedure used to assess the strength and functionality of specific muscles or muscle groups, particularly focusing on the hand in this context. This evaluation involves isolating a particular muscle or group of muscles and performing movements such as flexion, extension, abduction, or adduction while applying resistance, which can be provided by gravity or manual force. During the procedure, the patient is carefully positioned to facilitate the testing of the targeted muscle or muscle group, and stabilization may be necessary, often utilizing equipment such as railings, bars, or external support belts. The healthcare provider instructs the patient on the required movements, which may be demonstrated to ensure understanding. The assessment begins with passive range of motion, where the provider moves the patient's limb through the test movements to evaluate the range of motion and identify any signs of weakness or instability. Following this, the patient actively performs the movements against the applied resistance, allowing the provider to assess muscle strength. The findings from this evaluation are documented in a written report, which may include quantification of muscle function and strength using a standardized grading system, such as the Medical Research Council's Manual Muscle Testing Grades. It is important to note that different CPT® codes are designated for various muscle testing procedures: CPT® Code 95831 is used for muscle testing of the trunk or extremities excluding the hand, CPT® Code 95832 is specifically for testing the hand with or without comparison to the normal side, CPT® Code 95833 is for a total evaluation of the body excluding the hands, and CPT® Code 95834 is for a total evaluation of the body including the hands.
