1. Indications

The H-reflex, amplitude and latency study (CPT® Code 95934) is indicated for the evaluation of various conditions affecting the lower leg muscles. The following are specific indications for performing this procedure:

• Neuropathy Assessment of peripheral neuropathies that may affect the gastrocnemius and soleus muscles.
• Radiculopathy Evaluation of nerve root compression or injury that may impact the lower leg.
• Muscle Disorders Investigation of neuromuscular disorders that could lead to muscle weakness or dysfunction in the lower extremities.
• Reflex Activity Measurement of reflex activity to assess the integrity of the reflex arc involving the lower leg muscles.

2. Procedure

The procedure for conducting an H-reflex, amplitude and latency study involves several key steps to ensure accurate measurement and assessment of the gastrocnemius and soleus muscles:

• Step 1: Patient Preparation The patient is positioned comfortably, typically in a supine or seated position, to allow for easy access to the lower leg. The skin over the gastrocnemius and soleus muscles is cleaned to reduce impedance and improve electrode contact.
• Step 2: Electrode Placement Surface electrodes are placed over the target muscles (gastrocnemius and soleus) and the appropriate nerve, usually the tibial nerve, is identified. The placement of these electrodes is crucial for obtaining accurate readings of the H-reflex response.
• Step 3: Stimulation A mild electrical stimulus is applied to the tibial nerve, which elicits the H-reflex. The intensity and duration of the stimulus are carefully controlled to ensure patient comfort while obtaining reliable data.
• Step 4: Recording Responses The electrical activity of the gastrocnemius and soleus muscles is recorded using electromyography (EMG) equipment. The amplitude and latency of the H-reflex are measured, providing critical information about the nerve conduction and muscle response.
• Step 5: Data Analysis The recorded data is analyzed to assess the amplitude and latency of the H-reflex. This analysis helps in determining the functional status of the peripheral nervous system and identifying any abnormalities.

3. Post-Procedure

After the H-reflex study is completed, the patient may be monitored briefly to ensure there are no immediate adverse effects from the electrical stimulation. Typically, there are no significant post-procedure care requirements, and patients can resume normal activities shortly after the study. However, it is advisable to provide the patient with information regarding any potential mild discomfort or transient muscle soreness that may occur following the procedure. The results of the study will be documented and analyzed by the healthcare provider to guide further diagnostic or therapeutic interventions as necessary.
