Under supervision of the Senior RN Care Coordinator (SRNCC), the LPN Specialist provides patient care in home and office settings. Responsibilities include identifying and reporting patient nursing findings, following infection control procedures, recognizing and responding to patient or caregiver crises, adjusting care plans as needed, assisting with phone triage, updating physicians on patient status, transcribing and documenting physician orders (co-signed by RN), performing routine LPN and Hospice Aide assignments, supporting activities of daily living, administering specialized treatments authorized by physicians, assisting with referrals and patient admissions, completing documentation timely, participating in team meetings, and complying with agency standards and education requirements.
Minimum qualifications: current NY nursing license, preferred 1 year skilled nursing/home care/hospital experience, preferred 6 months hospice experience, ability to perform patient transfers/movements, BLS certification, effective communication skills, experience with EMR preferred, valid driver’s license and automobile insurance, proficiency in medication administration exam, availability by phone.
Current license and registration to practice nursing in New York State. Preferred one year experience in skilled nursing facility, home care, or hospital. Preferred six months hospice experience. Ability to perform one-person transfers and repetitive upper extremity movements. BLS certification. Effective verbal and written communication skills. Experience with electronic medical record (EMR) documentation preferred. Computer literacy. Valid driver's license and automobile with insurance. Successful completion of medication administration proficiency exam. Availability by telephone.
