Medical Records Coder Associate

UT Health San Antonio

Under direct supervision, responsible for reviewing inpatient and outpatient coding, ensuring coding compliance with federal regulations, and maintaining up-to-date coding guidelines and policy changes. Performs tasks required to facilitate medical billing, including abstracting complex patient-related data from medical records and coding diagnoses and procedures using ICD-10 and CPT classification systems.

This is a hybrid position with remote and on-campus work. The selected candidate must live within commuting distance of UT Health San Antonio. Onsite attendance is required for orientation and training. Transition to remote work depends on meeting productivity and quality standards, and remote coders may occasionally be required to attend on-campus training and meetings.

Responsibilities

  • Review, interpret, and assign diagnostic and procedural codes based on medical record documentation and correct coding principles.
  • Provide technical work in documentation and coding for medical billing.
  • Abstract patient-related data from medical records and code diagnoses and procedures using ICD-10 and CPT codes.
  • Perform coding-related charge review and claim edits daily to ensure timely and accurate billing.
  • Obtain medical records and charge fee information from patient care areas.
  • Contact other facilities to obtain medical records and billing information.
  • Verify charge capture and physician notes for completeness.
  • Abstract and enter relevant medical information from medical records, check for required signatures, and ensure proper documentation guidelines are followed.
  • Review charge documents for completeness.
  • Perform other duties as assigned.
Requirements & Qualifications

Qualifications

  • Proficiency in ICD-10 and CPT coding.
  • Basic understanding of medical terminology, anatomy, and physiology.
  • Meticulous attention to detail and accuracy.
  • Strong customer service, interpersonal, verbal, written, and communication skills.

Education

  • High school diploma or GED required.

Experience

  • None required.

License and Certification

  • Accreditation from a professional coding organization such as AHIMA, AAPC, RHIT, RHIA, or CCS certification is required.
