The Population Health Specialist uses knowledge of HEDIS quality metrics to complete documentation gap closures for payer programs using data collected from patients' electronic medical records and information submitted by clinics. This role identifies care gaps, coordinates with team members, requests patient records when needed, updates electronic medical records, ensures timely data submission to payers, tracks and reports gap closures, and supports clinic trainings and quality program materials.
The position also supports practice improvement efforts, works with multiple payer platforms, collaborates with internal teams on patient attribution and contract performance, and uses healthcare data, chart reviews, and evidence-based practices to improve quality performance across the network.
Qualifications
- High school diploma or GED required
- Bachelor's degree in healthcare preferred
- 2–3 years of healthcare experience in an outpatient medical practice preferred
- Proficiency with computer applications, especially Microsoft Office
- Experience with electronic medical records preferred
- Experience building training materials and training staff preferred
- 1–2 years of experience with lean or Six Sigma methodologies
- Experience with payer quality programs preferred
