Responsible for evaluating medical records and documentation, providing clinical abstracts, and assigning appropriate clinical diagnosis and procedure codes in accordance with nationally recognized guidelines.
Works collaboratively with team members to support quality improvement initiatives through team participation, data collection, process change implementation, and related activities.
Maintains confidentiality and protects sensitive patient, proprietary, and personnel information at all times.
Adheres to organizational and department safety standards and guidelines.
Essential Functions
- Analyze medical information from medical records and accurately code diagnostic and procedural information in accordance with national coding guidelines and reimbursement requirements.
- Provide thorough, timely, and accurate assignments of ICD and/or CPT4 codes.
- Assist in promoting accurate diagnostic information and complete documentation by physicians, nurses, and other professionals.
- Consult with medical providers and clinical staff to clarify missing or inadequate records and refer inconsistent documentation for clarification when needed.
- Provide the clinic with ICD, CPT, and/or HCPC updates quarterly or as needed.
- Perform other duties as required and assigned.
Knowledge, Skills, and Abilities
- Knowledge of anatomy, physiology, and medical terminology.
- Ability to code records using established coding guidelines and resources.
- Ability to review PTFs to ensure all codes are current.
- Ability to understand and meet coding deadlines to ensure timely bill submission.
- Ability to input abstract data and codes into computer systems for administrative and clinical reporting.
- Ability to audit physician coding for errors and discrepancies.
- Ability to maintain confidentiality of patient, personnel, and corporate data.
- Proficiency in Microsoft Office Suite and relevant software systems.
- Strong time management skills with the ability to meet deadlines.
- Strong organization and attention to detail.
- Ability to operate a personal computer, calculator, fax machine, copier, and other office equipment.
Minimum Qualifications
- High school diploma or GED, or Associate's degree or equivalent from a two-year college or technical school.
- Minimum of 1 year of related experience.
- CPC or CCS certification required.
- Must maintain a current Arizona Department of Public Safety Level 1 Fingerprint Clearance Card.
