Performs ongoing review and monitoring of electronic health record documentation to ensure compliance with documentation integrity standards.
Conducts audits, analyzes trends, facilitates corrective actions, and collaborates with providers, CDI, Coding, Compliance, and operational leaders to improve the accuracy, completeness, reliability, and integrity of the legal health record.
Supports documentation quality initiatives, chart corrections, amendments, record completion activities, and education related to documentation standards and EHR use.
Requirements & Qualifications
Qualifications
- High school diploma or equivalent required; associate's degree preferred
- Prior experience in a related field preferred
- Previous clerical experience in a hospital setting highly preferred
- Strong knowledge of medical terminology, documentation standards, and regulatory requirements
- Knowledge of HIPAA and federal/state regulations regarding release of protected health information
- Familiarity with regulatory and governing agencies including CMS, IDPH, HHS, HIPAA, and DNV
- Proficiency with Microsoft Office Suite and electronic health record systems
- Strong communication, organization, attention to detail, and critical thinking skills
- Ability to handle confidential information with discretion
- RHIT or CPC certification highly desirable
