The procedure described by CPT® Code 0431T involves the removal and replacement of a neurostimulator system specifically designed for the treatment of central sleep apnea. Central sleep apnea is a condition characterized by the temporary cessation of breathing due to a lack of signals from the brain to the muscles responsible for respiration. This condition can be triggered by various factors, including neurological issues such as stroke, anatomical abnormalities affecting the neck or brainstem, obesity, and the use of narcotic medications. The neurostimulator system utilized in this treatment comprises an implantable pulse generator, a stimulation lead that interfaces with the hypoglossal nerve (the twelfth cranial nerve), and a pressure sensing lead that is positioned in the chest. During the procedure, the focus is on the pulse generator, which is surgically accessed through an incision made in the chest wall. This incision allows the surgeon to expose the device, which is typically located in a pocket just beneath the pectoralis major muscle. The procedure entails disconnecting the stimulation and pressure sensing leads from the pulse generator, followed by the removal of the device itself. After the pulse generator is removed, the incision is closed. If a new pulse generator is to be inserted, it is placed in the same pocket, and the leads are reconnected and tested to ensure proper functionality.
