The procedure described by CPT® Code 27036 involves a surgical intervention known as a capsulectomy or capsulotomy of the hip. This procedure may also include the excision of heterotopic bone and the release of hip flexor muscles. A capsulectomy refers to the surgical removal of a portion of the joint capsule, while a capsulotomy involves making an incision in the joint capsule to access the hip joint. The hip joint capsule is a fibrous structure that surrounds the hip joint, providing stability and support. The procedure is performed through a skin incision located on the lateral aspect of the hip, extending from the greater trochanter to just below the lesser trochanter. During the operation, the fascia lata, a layer of connective tissue, is incised, and the vastus lateralis muscle is either divided or retracted to expose the underlying structures. The surgical team then identifies and releases the hip flexor muscles, which may include the gluteus medius, gluteus minimus, tensor fascia latae, rectus femoris, sartorius, and iliopsoas muscles, as necessary. This release is crucial for gaining access to the hip joint capsule. The procedure may also address heterotopic ossification, which is the abnormal formation of bone in soft tissue around the joint. This condition can significantly impair mobility and joint function, often resulting from trauma, neurological conditions, or prolonged immobility. The excision of any heterotopic bone is performed to restore normal range of motion and improve the patient's ability to perform daily activities.
