Thoracoscopy, surgical; with total pulmonary decortication, including intrapleural pneumonolysis (CPT® Code 32652) is a minimally invasive surgical procedure performed through the thoracic cavity. This technique, often referred to as video-assisted thoracoscopic surgery (VATS), allows for direct visualization and access to the pleural space. The procedure is indicated for patients with significant pleural disease, where a thickened fibrin layer, known as the rind or peel, encases the lung, impairing its function. In total pulmonary decortication, the surgeon removes this thickened layer from the entire lung, which may involve addressing adhesions that have formed between the lung and the chest wall. The approach typically involves making a small incision between the ribs, usually at the fifth or sixth intercostal space, to insert a thoracoscope and other surgical instruments.
