The CPT® Code 44205 refers to a surgical procedure known as a partial colectomy performed via laparoscopy, which involves the removal of a segment of the colon along with the terminal ileum, followed by the creation of an ileocolostomy. This minimally invasive technique begins with the establishment of pneumoperitoneum through a small incision near the umbilicus, allowing for the insertion of a trocar. Additional incisions are made in the upper and lower quadrants of the abdomen to facilitate the placement of more trocars, which are essential for the surgical instruments used during the procedure. The surgeon inspects the abdominal cavity to assess the condition of the colon and surrounding structures. In this procedure, the segment of the colon that is to be resected is mobilized, which means it is carefully freed from its attachments to allow for easier manipulation and removal. The distal resection site in the colon is identified, and the colon is divided at this point. An incision at one of the lower abdominal trocar sites is then enlarged to allow the proximal segment of the colon to be exteriorized, meaning it is brought out through the abdominal wall for surgical access. The proximal resection site in the ileum is also identified and resected using specialized tools such as clips and a harmonic scalpel, which aids in precise cutting and sealing of tissues. After the necessary segments of the bowel have been removed, the remaining portions are reconnected through a process called anastomosis, where the distal segment of the colon is sutured or stapled to the proximal segment of the ileum. This procedure is designed to restore continuity of the gastrointestinal tract while minimizing recovery time and complications associated with traditional open surgery.
