Common Language Description

The CPT® Code 44208 refers to a laparoscopic surgical procedure known as a partial colectomy with anastomosis, specifically involving a coloproctostomy (low pelvic anastomosis) and the creation of a colostomy. This procedure is performed through small incisions in the abdomen, utilizing a laparoscope, which is a specialized instrument that allows the surgeon to view the internal structures of the abdomen without the need for large incisions. The process begins with the establishment of pneumoperitoneum, which is the inflation of the abdominal cavity with gas to create a working space for the surgical instruments. The surgeon inspects the abdominal cavity, identifies the superior rectal vessels, and carefully dissects them to prepare for the resection of the affected colon segment. During the procedure, the rectum is mobilized, and a distal resection is performed using an endoscopic linear stapler, which facilitates the cutting and sealing of tissue. The segment of colon that is to be removed is then mobilized to allow for its exteriorization through a small incision. Following the resection, the remaining bowel is prepared for anastomosis, which is the surgical connection of the remaining segments of the colon and rectum. In this specific procedure, a diverting colostomy is created, which involves bringing a loop of colon through the abdominal wall to form a stoma. This stoma is then sutured to the skin, allowing for the diversion of fecal matter. The procedure concludes with the closure of the portal incisions and the placement of a colostomy appliance at the stoma site, ensuring that the patient has a functional means of waste elimination post-surgery.
