A flexible colonoscopy is a diagnostic and therapeutic procedure that allows for the examination of the entire colon, extending from the rectum to the cecum. This procedure is particularly significant when there is a need to control bleeding within the colon, utilizing various methods to achieve hemostasis. During the colonoscopy, a flexible tube known as a colonoscope is inserted into the rectum and carefully advanced through the colon. The use of air insufflation is critical as it helps to expand the colon, allowing for better visualization of the mucosal surfaces. This enhanced view enables the physician to inspect for any abnormalities, such as lesions, strictures, or signs of bleeding. When a bleeding site is identified, several techniques may be employed to control the bleeding. These methods include the application of thermal energy through devices such as bipolar or unipolar cautery, or a heater probe, which directly applies heat to the bleeding area. Additionally, an injection of epinephrine may be administered to constrict blood vessels and assist in stopping the bleeding. Non-contact methods, such as YAG laser coagulation and argon plasma coagulation, are also available for coagulating the bleeding site. In cases of tears or lacerations, staples or hemoclips may be utilized to bring the tissue margins together. After the intervention, the colonoscope is withdrawn, and a final inspection of the mucosal surfaces is conducted to ensure that no further abnormalities are present. This comprehensive approach not only aids in diagnosing conditions but also provides immediate therapeutic options to manage bleeding effectively.
