Revision or removal of intracranial neurostimulator electrodes

Common Language Description

The procedure described by CPT® Code 61880 involves the revision or removal of intracranial neurostimulator electrodes. This surgical intervention is performed to either reposition existing electrodes or to completely remove them from the patient's body. The process begins with making an incision in the scalp, which allows the surgeon to access the skull and the burr holes where the electrodes are placed. Burr holes are small openings drilled into the skull to facilitate the placement of the electrode wires that stimulate specific areas of the brain. The procedure may involve the removal of plastic caps that secure the electrode wires in place, allowing for either repositioning of the electrodes within the brain or their complete removal. In cases of revision, the electrodes may be adjusted to a new location within the brain, and the plastic caps are replaced to secure the wires in their new position. The incision in the scalp is then closed using sutures or staples. For partial removal, the electrode wire is coiled beneath the skin adjacent to the skull, and the skin is closed similarly. In instances of complete removal, a separate incision is made over the stimulator, typically located in the anterior chest area below the clavicle. The electrode wire is disconnected from the neurostimulator device and extracted through a tunnel that connects the cranium to the chest. After the removal, both incisions are closed with sutures or staples. This procedure is critical for patients who require adjustments to their neurostimulator system or who no longer need the electrodes in place.
