The procedure described by CPT® Code 63661 involves the percutaneous removal of spinal neurostimulator electrode arrays, which are used in spinal cord stimulation systems to manage chronic back and/or leg pain. These systems work by delivering electrical impulses to the spinal cord, which can help alleviate pain by activating neurons that inhibit pain signals and creating a tingling sensation that can mask pain sensations. Initially, a temporary electrode array is implanted to assess the effectiveness of the stimulation in relieving pain. If the temporary device proves effective, it may be replaced with a permanent device; if not, it is removed without replacement. The removal process for the electrode array, as outlined in this code, includes the use of fluoroscopy to guide the procedure. The surgeon makes a small incision in the skin over the insertion site, exposes the vertebra, and locates the electrode array within the epidural space. The leads are then carefully dissected free from the subcutaneous tunnel and removed. If a permanent electrode array is to be placed, this would be done in a separate procedure. If not, the incisions are simply closed after the removal of the temporary device.
