The procedure described by CPT® Code 63662 involves the removal of a spinal neurostimulator electrode plate or paddle that has been previously implanted via a surgical technique known as laminotomy or laminectomy. This procedure is typically performed to alleviate chronic back and/or leg pain through the use of an implantable spinal cord stimulation system. The system works by delivering electrical stimulation to the spinal cord, which activates pain-inhibiting neurons and creates a tingling sensation that helps mask pain sensations. Initially, a temporary electrode array, plate, or paddle is placed to assess the effectiveness of the stimulation in managing pain. If the temporary device proves effective, it may be replaced with a permanent device; if not, it is removed without replacement. In the context of CPT® Code 63662, the removal process involves several critical steps. The procedure begins with the opening of the subcutaneous pocket that houses the generator or receiver, followed by the disconnection of the leads. Fluoroscopic guidance may be utilized to assist in the precise location of the electrode plate or paddle. An incision is made over the spine, and the overlying soft tissue is carefully dissected to expose the lamina. The electrode plate or paddle is then located within the epidural space and removed. The leads are also dissected free from the subcutaneous tunnel and extracted. If a permanent electrode plate or paddle is to be implanted, this will be conducted as a separate, reportable procedure. If no replacement is necessary, the incisions are subsequently closed. This detailed process ensures that the removal is conducted safely and effectively, minimizing potential complications for the patient.
