The procedure described by CPT® Code 65779 involves the placement of a single layer of amniotic membrane on the ocular surface, utilizing sutures for secure attachment. Amniotic membrane is a biological tissue derived from the placenta, known for its healing properties, particularly in ocular surgeries. This membrane is harvested, dried, and prepared for application to promote wound healing in the eye. Prior to the procedure, the eye is irrigated with normal saline, and antibiotic drops may be administered as needed for a duration of 24 to 48 hours to prevent infection. Anesthetic eye drops or a facial nerve block is employed to ensure patient comfort during the procedure. To facilitate the application of the amniotic membrane, an eye speculum is used to keep the eye open. The membrane, which is cut to fit the size of the conjunctival sac, is carefully placed over the affected area of the eye. It is crucial to avoid covering the entire cornea, as this could lead to temporary hazy vision. The application technique involves spreading the membrane from the upper fornix while the patient looks down, and then over the lower fornix as the patient looks up. After the membrane is positioned, the eye speculum is removed, and liquid paraffin is applied to seal the eye closed. Both eyes are then bandaged for 24 to 48 hours to allow for absorption of the amniotic membrane. The sutured placement of the membrane, as indicated by CPT® Code 65779, ensures that the graft remains securely in place, with sutures being removed three days post-procedure to facilitate healing.
