Common Language Description

The procedure described by CPT® Code 65781, known as ocular surface reconstruction via limbal stem cell allograft, is a surgical intervention aimed at addressing significant damage to the cornea, which may result from traumatic injury or various disease processes. This procedure is specifically indicated for patients who exhibit severe impairment of the ocular surface, leading to an inability of the eye to heal naturally. Limbal stem cells, which are derived from the limbus—the white outer edge of the eyeball—play a crucial role in the regeneration of corneal epithelial cells. These cells form the outermost protective layer of the cornea, serving as a barrier against pathogens and environmental contaminants. In cases where a living donor is utilized, the limbal stem cells are meticulously harvested in a strip from the donor's eye, which is a separate procedure that must be reported independently. Alternatively, the procedure may involve the use of previously harvested limbal stem cells from a cadaver. During the surgical process, the damaged corneal epithelium is carefully debrided to eliminate the compromised epithelial stem cells, and the transplant bed is meticulously prepared to receive the allograft. The conjunctiva is resected, exposing the sclera to a designated area beyond the limbus, allowing for the precise placement of the limbal stem cell allograft, which is then secured in position with sutures. Post-operative care includes the application of antibiotic and corticosteroid ointments to the surgical site, followed by the protection of the eye with a patch and an eye shield to facilitate healing.
