Entropion is a medical condition characterized by the inward rotation of the eyelid margin towards the surface of the eye. This abnormal positioning causes the eyelashes and the eyelid itself to come into contact with the conjunctiva and cornea, leading to irritation and discomfort. If left untreated, this irritation can escalate to more severe complications such as corneal abrasion and ulceration, which can significantly impact vision. Entropion is most frequently observed in the lower eyelid of elderly individuals, making it a common concern in geriatric ophthalmology. The procedure associated with CPT® Code 67921 involves a suture repair technique aimed at correcting this condition. During the procedure, a local anesthetic is administered to numb the eyelid, ensuring patient comfort. The surgical technique involves placing paired full-thickness sutures from the conjunctival side of the eyelid, just below the tarsal border, and exiting through the skin of the eyelid. The sutures are strategically angled to ensure that the exit point is closer to the lid margin, which facilitates the eversion of the inverted eyelid. The ends of these sutures are then tied and left in place for a duration of up to two weeks. It is important to note that this suture repair is generally considered a temporary solution, serving as a preliminary measure until a more definitive and permanent repair can be performed.
