Radiopharmaceutical therapy by intra-articular administration involves the direct injection of a radiolabeled compound, such as yttrium, gold, or phosphorus, into a joint space. This therapeutic approach is primarily utilized to manage recurrent bleeding that occurs as a result of chronic synovitis, particularly in patients diagnosed with hemophilia. The procedure is typically performed on major joints, including the elbows, knees, and ankles, and is often conducted in an outpatient setting, such as a nuclear medicine clinic or facility. Local anesthesia is generally sufficient for pain management during the injection process. However, in cases where the hip or shoulder joints are targeted for injection, the procedure may necessitate the use of radiological guidance to ensure accurate placement of the radiopharmaceutical. In such instances, patients may require hospitalization and the administration of sedation or general anesthesia to facilitate the procedure safely and effectively.
