Official Description

Right heart catheterization including measurement(s) of oxygen saturation and cardiac output, when performed

Common Language Description

The procedure described by CPT® Code 93451 refers to a right heart catheterization, a diagnostic procedure used to assess the function of the right side of the heart and the pulmonary arteries. This procedure involves the insertion of a catheter into the right femoral vein, which is typically accessed in the groin area. The skin over the access site is first cleansed to reduce the risk of infection, and a local anesthetic is administered to minimize discomfort during the procedure. A small incision is made to facilitate the insertion of a needle into the blood vessel, followed by the placement of a sheath that allows for the introduction of a guidewire. The guidewire is carefully threaded through the venous system, passing through the femoral vein, external iliac vein, and inferior vena cava, ultimately reaching the right atrium of the heart. Once the guidewire is in place, a catheter is advanced over it into the right atrium, and then further into the right ventricle and pulmonary arteries. During this process, the right heart chambers are visually inspected, and critical measurements such as pressures within these chambers, oxygen saturation levels, and cardiac output are obtained. The procedure also includes an evaluation of the tricuspid and pulmonary valves. In some cases, a separately reportable angiogram of the right heart and/or pulmonary arteries may be performed to provide additional diagnostic information. After the necessary measurements and inspections are completed, the catheter is withdrawn, and compression is applied to the venous access site to prevent bleeding, followed by the application of a dressing to protect the site.
