Debridement (eg, high pressure waterjet with/without suction, sharp selective debridement with scissors, scalpel and forceps), open wound, (eg, fibrin, devitalized epidermis and/or dermis, exudate, debris, biofilm), including topical application(s), wound assessment, use of a whirlpool, when performed and instruction(s) for ongoing care, per session, total wound(s) surface area; first 20 sq cm or less

The procedure described by CPT® Code 97597 involves the debridement of an open wound, which is a critical step in wound management. Debridement is the process of removing dead, damaged, or infected tissue to promote healing and prevent infection. In this context, the procedure can utilize various methods, including high-pressure waterjet systems or sharp instruments such as scissors, scalpels, and forceps. The primary goal is to eliminate non-viable tissue, which may include fibrin, devitalized epidermis and/or dermis, exudate, debris, and biofilm, all of which can impede the healing process. Before the debridement, a thorough assessment of the wound is conducted to evaluate its size, depth, and any signs of inflammation, ulceration, or necrosis. A whirlpool may be employed to cleanse the wound and prepare it for debridement. The high-pressure waterjet system is particularly effective as it combines the force of water with suction to simultaneously cut and aspirate the target tissue, ensuring a more efficient removal of unwanted material. Following the debridement, topical agents may be applied to facilitate healing, and appropriate dressings are used to protect the wound. Additionally, instructions are provided to the patient or caregiver regarding ongoing care, including how to change dressings and monitor the wound for any changes. This procedure is billed per session for the first 20 square centimeters of the wound surface area, with additional charges applicable for larger areas as indicated by CPT® Code 97598.
