Debridement (eg, high pressure waterjet with/without suction, sharp selective debridement with scissors, scalpel and forceps), open wound, (eg, fibrin, devitalized epidermis and/or dermis, exudate, debris, biofilm), including topical application(s), wound assessment, use of a whirlpool, when performed and instruction(s) for ongoing care, per session, total wound(s) surface area; each additional 20 sq cm, or part thereof (List separately in addition to code for primary procedure)

The procedure described by CPT® Code 97598 involves the debridement of an open wound, which is a critical step in wound management. Debridement is the process of removing dead, damaged, or infected tissue to promote healing and prevent infection. This procedure can utilize various methods, including high-pressure waterjet systems or sharp instruments such as scissors, scalpels, and forceps. The primary goal is to clean the wound by removing fibrin, devitalized epidermis and/or dermis, exudate, debris, and biofilm, which can impede the healing process. Before the debridement, a thorough assessment of the wound is conducted to evaluate its size, depth, and any signs of inflammation, ulceration, or necrosis. In some cases, a whirlpool may be employed to prepare the wound for debridement, enhancing the effectiveness of the procedure. The high-pressure waterjet system is particularly noteworthy; it consists of a power console, foot pedal, and a disposable hand-held waterjet/aspirator that delivers a focused stream of sterile water to the wound. This method not only cleans the wound but also aspirates the removed tissue into a receptacle, ensuring a thorough debridement process. Following the removal of non-viable tissue, topical agents may be applied to facilitate healing, and appropriate dressings are used to protect the wound. Additionally, the patient or caregiver receives instructions on how to care for the wound, including guidance on dressing changes and ongoing wound evaluation. It is important to note that CPT® Code 97598 is used for each additional 20 square centimeters of wound surface area debrided, or part thereof, following the initial 20 square centimeters covered by CPT® Code 97597.
