The Patient Access & Intake Coordinator plays a key role in facilitating a smooth and efficient patient admission process. This position provides vital administrative and operational support to the Business Development team and the Central Access and Authorizations Team (CAAT), ensuring timely initiation and management of referral activity.
Responsible for launching the referral management process, the coordinator oversees admission workflows, gathers and verifies required documentation, coordinates information for necessary approvals, and communicates essential information to relevant departments. Acting as a central liaison between Business Development, facility leadership, and CAAT colleagues, this coordinator manages admission logistics, confirms readiness to receive the patient, and ensures all clinical and operational details are aligned prior to arrival.
This role actively contributes to quality improvement, problem solving, and productivity initiatives within an interdisciplinary team model, demonstrating accountability and commitment to operational excellence.
Key responsibilities include intake of referrals, establishing patient accounts in referral management platforms, entering demographic information, coordinating bed assignments and admission paperwork, insurance verification, communicating patient needs for equipment and supplies, maintaining referral logs, and participating in professional development.
Qualifications include a High School Diploma or GED (Associates or Bachelors preferred), preference for healthcare-related education or experience, excellent communication skills, strong computer skills, attention to detail, and experience in healthcare or post-acute care preferred.

- Preference towards healthcare related concentration or licensed healthcare provider or equivalent experience
- Experience in healthcare related area preferred
- Work experience in post-acute care is a plus
- Excellent oral and written communication skills
- Strong computer skills and data entry with attention to detail
- Ability to work as a team player with professional attitude
- Must read, write, and speak fluent English
- Good and regular attendance
