Key responsibilities include facilitating new patient referral intake by acquiring, organizing, summarizing, and tracking medical records from all involved providers. The role demands accuracy in maintaining records to positively impact quality, cost, and outcomes of care. It involves close collaboration with the Care Navigation Team, understanding diverse practice styles of veterinary teams, and managing electronic medical records across systems. The specialist will also contribute to developing electronic tools to improve care provision efficiencies.
Qualifications require over 5 years of healthcare experience as a veterinary technician, nurse, or in relevant clinical roles. Experience in general practice and specialty/emergency veterinary medicine is preferred. The candidate must be detail-oriented, self-motivated, and comfortable with medical terminology, records, and diagnostic reports. Proficiency with EMR/PIMS systems and office software is essential. Strong communication skills and the ability to work independently while being a team player are necessary. Commitment to high-quality customer service and care excellence is expected.
