Medical Records Clerk

We are seeking a dependable and detail-oriented Medical Records Clerk to join our hospice care team. In this vital administrative role, you will manage and maintain accurate patient records, ensure compliance with healthcare regulations, and provide essential office support to help our team deliver exceptional care. Your work enables caregivers to focus on what matters most—making every moment count.

Responsibilities include maintaining and closing medical records per company policy; reviewing records for completeness and compliance; obtaining missing chart information including physician signatures; managing release of information to authorized parties with signed authorizations; copying, mailing, or delivering requested medical information accurately and timely; collaborating with clinical staff to support timely patient admissions; providing administrative support including phone answering, supply ordering, and data entry; conducting medical record audits; participating in patient care coordination and hospice quality assessment programs; purging closed case records and sending them to archives; maintaining a master patient index; promoting company core values; and completing required compliance training annually.

Qualifications require a high school diploma or equivalent and a minimum of three years experience in office work or a medical records department. Valid driver's license and current automobile insurance are required.

Strong organizational, record-keeping, filing, and typing skills are essential. Excellent oral and written communication, proficiency in documentation management, ability to operate office equipment effectively, flexibility, multitasking capability, and tactful interpersonal skills are needed.
