Under the direction of the Director of Clinical Quality, the Health Management Information Specialist is responsible for maintaining the security, confidentiality, completeness, and accuracy of medical records including Release of Information (ROI) requests and managing electronic documents (efaxes) according to organizational policies and procedures.
Follow HIPAA policies and laws. Verify that all release of information requests comply with Texas State law. Manage phone calls regarding medical records and ROI. Communicate effectively with patients and professional offices in person, electronically, and by phone.
Process all incoming and outgoing medical release forms with thorough documentation. Scan and append documents to patients' charts in eClinicalWorks (ECW). Prioritize workflow and process urgent items timely and accurately. Process medical records subpoenas with supervisor approval.
Coordinate medical records audit requests/notifications with supervisor, maintain logs and documentation. Follow up on records retrieval from external institutions. Participate in yearly review and revision of medical records protocols.
Review incoming faxes and assign documents to appropriate ECW folders. Assess areas of improvement, develop plans and evaluation criteria. Purge records per policy and retention requirements for Texas, licensure, and accreditation. Keep supervisor informed of issues.
Use computer systems, copiers, fax machines, scanning technology; prior experience with Electronic Health Records is a plus. Charge patients according to Medical Records Fee Schedule. Participate in Compliance, Risk and Safety, Quality Assurance, and Performance Improvement activities. Other duties as assigned.
High school diploma or equivalent; Medical records experience; Effective communication skills; Familiarity with medical terminology; Certification as Medical Records Specialist and/or Release of Information Specialist.
Graduate of approved high school or GED equivalent; Medical assistant certificate preferred, Certified Medical Assistant Certificate strongly preferred; Current Basic Life Support (BLS) or equivalent; Experience in Federally Qualified Health Centers preferred; Multi-lingual (English/Spanish) required; Flexible availability including evenings and weekends.
