Minimum of 1-2 years' experience in Medical Records (preferably in a Skilled Nursing or Long-Term Care setting).

Must possess a valid State Driver's License with a clean driving record.

Must be able to assist residents with transfers and push wheelchairs.

Proficiency with Electronic Health Records (e.g., PointClickCare) and Microsoft Office, as well as GPS (e.g., Google Maps, Apple Maps, Waze).

This dual role position requires managing the integrity, confidentiality, and accessibility of resident health records while transporting residents to and from outside medical appointments.

Maintain resident medical records in accordance with federal and state regulations (HIPAA, CMS) and facility policies.

Perform regular audits of charts and electronic health records (EHR) to ensure completeness, accuracy, and timely physician signatures.

Process new resident records and ensure discharge files are closed, indexed, and stored securely.

Manage all requests for medical records from legal entities, families, and other healthcare providers.

Maintain the medical records room and digital filing systems for quick retrieval.

Safely transport residents to and from scheduled doctors' appointments, dialysis, or specialized clinics using the facility vehicle.

Coordinate with nursing and external clinics for weekly transport schedules.

Assist residents in and out of vehicles (including wheelchair lift operation) and ensure safe check-in for appointments.

Act as liaison, delivering paperwork or instructions from outside physicians back to the facility's clinical team.

Requirements & Qualifications

Minimum 1-2 years experience in Medical Records, preferably in Skilled Nursing or Long-Term Care. Valid State Driver's License with clean record. Ability to assist residents with transfers and push wheelchairs. Proficiency with Electronic Health Records systems (e.g., PointClickCare), Microsoft Office, and GPS applications.
