Perform clerical duties associated with obtaining, completing, and maintaining patient medical records for 20 hours a week. Serve as a front desk clerk for 50% of the week. Weekend and holiday work is required with possible rotation.

Essential Functions:
- Sort, file, and collate various medical records and information such as progress notes, treatment plans, nursing/clinical notes, and discharge summaries into patient medical records.
- Create medical record files.
- Ensure medical records are complete, accurate, and timely.
- Research lost or missing records/information per established procedures.
- Respond to requests for medical records from outside agencies and third-party sponsors.
- Communicate with transcriptionists or transcription vendors to resolve issues or errors regarding reports.
- Assist staff in locating records in the medical records department.
- Maintain accurate logs, card files, statistics, and information release forms.
- Ensure records are complete before filing or re-filing and update logs accordingly.
- Perform medical record audits.
Other duties as assigned.
Requirements:
- High school diploma or equivalent.
- Preferred experience in quantitative medical record reviews.
