At Diversicare, we're looking for a passionate Director of Care Coordination to join our exceptional team. This role is dedicated to making a difference in the lives of our patients and residents.

As the Director of Care Coordination, you will play a pivotal role in aligning care resources with patient needs, benefits, and exceptional service. You will embody and shape a culture of trust, respect, customer focus, compassion, diplomacy, appreciation, and strong communication skills.

Responsibilities include facilitating daily care coordination meetings, managing the restorative nursing program, ensuring timely communication with patients and families about condition changes and discharge planning, collaborating with the clinical team to develop care plans, coordinating discharge planning and post-discharge communication, ensuring accurate care documentation per Medicare and Managed Care guidelines, reviewing Additional Documentation Requests, coordinating the RAI process for compliance, conducting care plan conferences, providing education on RAI Process and MDS coding, monitoring quality measures, maintaining documentation, and ensuring regulatory compliance.

Qualifications:

  • Degree in nursing and current Registered Nurse license
  • Two years of MDS experience with knowledge of healthcare reimbursement, utilization management, and discharge planning
  • Experience in strategic leadership, team management, and project leadership
  • Ability to formulate and implement strategic plans
  • Strong verbal, written, interpersonal, communication, and organizational skills
  • Knowledge of PDPM requirements and regulatory standards
  • Previous case management experience preferred
Requirements & Qualifications

  • Experience leading teams and managing projects
  • Strong communication and organizational skills
  • Knowledge of PDPM and regulatory requirements
  • Preferably previous case management experience
