Responsibilities include:

• Complete MDS Assessments
• Facilitate daily care coordination meetings
• Participate in Engage/72 HR Admission meetings
• Manage restorative nursing program
• Ensure timely communication with patients and families
• Collaborate on care plans and discharge planning
• Monitor Quality Measures
• Serve as skilled care documentation specialist ensuring Medicare and Managed Care compliance
• Review Additional Documentation Requests (ADRs)

Qualifications:

• Nursing degree and Registered Nurse license
• Experience in MDS
• Knowledge of healthcare reimbursement, utilization management, discharge planning
• Strategic leadership and team/project management experience
• Strong communication, organizational, and interpersonal skills
• Knowledge of PDPM insurer requirements and regulatory compliance
• Preferred: case management experience
