Health Home Intake Specialist - Chinese-Speaking (Per Diem)

Join a mission-driven team supporting New Yorkers in the community by conducting in-person member enrollment visits and assessments throughout assigned communities. This flexible per diem role involves face-to-face intake and renewal visits in members' homes or community settings, ensuring timely support, accurate assessments, and smooth transitions into services.

Key responsibilities include conducting comprehensive assessments (medical, behavioral health, psychosocial, social determinants of health), obtaining consent and care plan signatures, verifying identification and insurance information, educating members on program services, and maintaining compliance and documentation quality.

This is a fully field-based role requiring local travel and independent work. Structured training, ongoing support, mileage and transportation reimbursement are provided.

Compensation is per completed assignment: $250 per intake package and $100 per renewal visit.

Qualifications:

  • High School Diploma or GED required
  • Prefer candidates with associate's or bachelor's degree in Human Services, Social Work, Public Health, Healthcare Administration, or related field
  • Experience in intake coordination, healthcare enrollment, care management, social services, or community outreach preferred

Ideal candidates are compassionate, detail-oriented, compliance-conscious, reliable, and skilled at building rapport with diverse populations.
