Responsible for detailed preview of charts for high complexity patients and patients in the population health programs within the virtual care center. Collaborates with providers across the organization to ensure accurate coding for diagnosis. Assists in providing cost-effective healthcare services through telephonic outreach. Acts as a liaison between patients, physicians, and interdisciplinary healthcare team members.
Reviews charts to identify ongoing health maintenance needs and facilitates next steps to assist patients in meeting those needs. Obtains outside medical records and assists with uploading into the electronic medical record. Schedules patient appointments and assists licensed personnel to enhance comprehensive patient care and reduce gaps in care. Maintains constant alertness and works in a safe manner. Performs other duties as assigned.
Bachelor's degree in a healthcare related field or equivalent combination of education and experience. No prior experience required.
Physical requirements include frequent standing, walking, hearing, speech communication, lifting/carrying up to 25 lbs frequently, reaching, gripping, keyboard use/data entry, vision for near and far distances, occasional bending, stooping, crawling, kneeling, sitting, squatting, repetitive foot/leg and hand/arm movements, occasional lifting/moving patients, occasional use of smell, and occasional driving.
No professional license or certification required.
