Job Summary

A Medical Coder, or Certified Professional Coder, reviews patient medical records after a visit and translates the information into codes used by insurers to process claims. The role includes confirming treatments with medical staff, identifying missing information, and submitting forms to insurers for reimbursement.

Responsibilities

Assign codes to medical procedures and diagnoses
Ensure codes are assigned correctly and sequenced appropriately according to government and insurance regulations
Comply with medical coding guidelines and policies
Review patients' charts and documents for verification and accuracy
Follow up on and clarify unclear information with staff members
Collect information from physician documentation and other sources to prepare monthly reports
Implement strategic procedures and evaluation methods that support accurate results
Examine reported medical malpractice cases by analyzing medical procedures, diagnoses, or events related to the negligence
