Abstract clinical information from medical records, charts, and documents and assign appropriate ICD-9/ICD-10 and/or CPT-4 codes to patient records according to established procedures. Work with coding databases, confirm DRG assignments, and support accurate reimbursement and compliance.
Responsibilities include coding patient records using computerized systems, abstracting data for statistical reporting, identifying complications and comorbidities, correcting coding errors, responding to physician documentation inquiries, and performing other duties as assigned.
High school diploma or GED, or completion of a certificate program in medical coding and billing.
Required experience:
- Six months of experience in coding or medical records using ICD and CPT coding systems
