At Duke Health, this role supports the Patient Revenue Management Organization, a centralized revenue cycle team responsible for scheduling, registration, coding, billing, and other revenue functions across the health system.

The Medical Records Coder II reviews complex medical records, assigns accurate diagnosis and procedure codes, and helps ensure appropriate reimbursement for hospital and professional charges. The role also includes coordinating the work of other employees, supporting training and continuing education, and helping maintain coding quality through audits and documentation review.
Key responsibilities include:
- Reviewing medical records for primary and secondary diagnoses and procedures
- Applying ICD-10-CM, CPT, and CPT-4 coding conventions
- Sequencing diagnoses and procedures according to coding guidelines
- Ensuring accurate DRG and APC assignment
- Abstracting and compiling data for reimbursement purposes
- Educating physicians and communicating with clinical teams to obtain necessary documentation
- Assisting with special projects and other related duties
Must hold one of the following active certifications:
- RHIA
- RHIT
- CCS
- CPC
- HCS-D
Experience requirements:
- RHIA: no experience required
- RHIT: no experience required
- CCS: 1 year of coding experience required
- CPC or HCS-D: 2 years of coding experience required
Additional qualifications and skills:
- High school diploma required
- Advanced ICD-10-CM and CPT-4 coding knowledge
- Strong understanding of anatomy and physiology
- Medical terminology knowledge
- Familiarity with DRG and APC reimbursement concepts
- Coding software experience
- Effective written and verbal communication skills
- Data entry / CRT skills
