Medical Records Technician - Inpatient/Outpatient Coder

CVP (Customer Value Partners)

CVP is seeking Medical Records Technicians (Inpatient/Outpatient Coders) to provide medical coding services in support of the Captain James A. Lovell Federal Health Care Center (FHCC) in North Chicago, Illinois.

This is a fully remote position with working hours of Monday through Friday, 7:30 AM to 4:00 PM CST. The FHCC serves more than 75,000 Veterans from northeastern Illinois and southeastern Wisconsin, with 354 inpatient beds and approximately 215,000 inpatient admissions and outpatient visits annually.

Responsibilities include:

  • Review provider medical record coding for completeness and accuracy, and clarify or correct coding as needed.
  • Query providers using encrypted Outlook email and the Provider Query Foundation OneNote to ensure accurate coding in compliance with VHA HIM Practice Brief standards.
  • Abstract medical, surgical, laboratory, pharmaceutical, radiologic, demographic, social, and administrative data from the medical record.
  • Identify, document, sequence, and code diagnoses and procedures accurately.
  • Determine appropriate codes for routine, new, and unusual diagnoses and procedures not clearly listed in ICD-10-CM, ICD-10-PCS, CPT, and HCPCS.
  • Ensure medical records are fully reviewed so that conditions and care rendered are properly documented, coded, and sequenced in accordance with coding compliance guidelines and the National Correct Coding Initiative.
  • Confirm physician documentation supports coded diagnoses and procedures, contacting clinicians when needed.
  • Concurrently code inpatient encounters when not completing discharged cases, for inpatient coders.
  • Use encrypted Outlook email and 3M/Solventum 360 for clarification of conflicting or ambiguous information.
  • Maintain coding accuracy at 95% or greater.
  • Maintain diagnostic and procedural statistics to support appropriate reimbursement from third-party payers.
  • Stay current on regulatory changes affecting coded information from CMS and other regulatory bodies.
  • Comply with HIPAA, VHA Coding Guidelines, and applicable civilian and VA directives, including Joint Commission, AMA, AHA, and CMS standards.
  • Protect privacy and confidentiality of patient records and sensitive information.
Requirements & Qualifications

Qualifications

  • Ability to successfully undergo a government-sponsored background investigation.
  • U.S. citizenship required.
  • Proficiency in spoken and written English.
  • Associate's degree from an accredited college or university in health information technology, health information management, or a related field with at least 12 semester hours in health information technology/health information management; bachelor's degree preferred.
  • Minimum of 3 years of continuous coding experience in a facility serving a patient population equal to or exceeding the VA's current patient population.
  • Active certification in at least one of the following:
    • RHIT (AHIMA)
    • CCS or CCS-P (AHIMA)
    • RHIA (AHIMA)
    • CPC (AAPC)
  • Knowledge of Oracle Cerner, 3M/Solventum, ICD-10-CM, ICD-10-PCS, CPT, Evaluation and Management coding, and HCPCS.
  • Strong knowledge of MS-DRG logic, CC/MCC capture, POA indicators, and NCCI guidelines.
  • Familiarity with medical terminology, anatomy, physiology, and healthcare documentation standards.
  • Experience with inpatient surgical coding, including multiple procedures, staged procedures, revisions, returns to OR, and device replacements, preferred.
  • Prior VA or federal healthcare system coding experience preferred.

  • Hourly pay: $30.00/hr
  • Health & Wellness (H&W): $5.09/hr
  • H&W is prorated based on enrollment in offered benefits, including automatic enrollment in employer-paid plans.
