Medical Records Coder - Senior

UT Health San Antonio

Under direct supervision, responsible for reviewing inpatient and outpatient coding, ensuring coding compliance with federal regulations, and maintaining up-to-date coding guidelines and policy changes. This role supports medical billing by abstracting complex patient data from medical records and coding diagnoses and procedures using ICD-10 and CPT classification systems.

This is a hybrid position with remote and on-campus work. The candidate must live within commuting distance of UT Health San Antonio. Onsite attendance is required for orientation and training, and ongoing remote work depends on productivity and quality standards. Remote coders may occasionally be required to attend on-campus training and meetings.

Responsibilities

  • Review, interpret, and assign diagnostic and procedural codes based on medical record documentation and correct coding principles.
  • Perform documentation and coding tasks for medical billing, including abstraction of patient-related data from medical records.
  • Work coding-related charge review and claim edits daily to support timely and accurate billing.
  • Obtain medical records and charge fee information from patient care areas.
  • Contact other facilities to obtain records and information needed for billing services rendered.
  • Verify charge capture and physician notes for completeness.
  • Check for required signatures and ensure documentation guidelines are followed.
  • Code diagnoses and procedures using classification coding systems.
  • Review charge documents for completeness.
  • Perform other duties as assigned.
Requirements & Qualifications

Qualifications

  • Proficiency in ICD-10 and CPT coding.
  • In-depth understanding of medical terminology, anatomy, and physiology.
  • Meticulous attention to detail and accuracy.
  • Strong customer service, interpersonal, verbal, written, and communication skills.
  • Ability to work effectively with internal and external customers in a timely and respectful manner.
  • Accreditation or certification from AHIMA, AAPC, RHIT, RHIA, or CCS is required.

Education

  • High school diploma or GED required.

Experience

  • Five years of experience in medical record abstraction and coding required.
